Please do not send medical information through this website. The contact form is not a secure channel for health information. We collect what we need about your health securely, at your consultation.
This policy explains what USC MedSpa, operated by Cimikoski Stem Cells, LLC (“we”, “us”), collects through this website and what we do with it.
What we collect
- What you send us. When you submit the consultation form we receive your name, phone number, email address, the area of interest you selected, and anything you write in the optional message field.
- Technical data. Like most websites, our server and our analytics record information such as your IP address, browser, device type, the pages you viewed and the site or ad that brought you here.
- Cookies and similar technologies. We use cookies for basic site function, for analytics, and for advertising measurement.
We do not knowingly collect personal information from children under 13.
How we use it
- To respond to your request and arrange a consultation.
- To contact you by phone, SMS and email about that request, where you have consented.
- To measure how our advertising performs, in aggregate.
- To keep the website secure and working.
We do not sell your personal information.
Messages and how to stop them
If you tick the consent box, you agree to be contacted by phone, SMS and email about your request. Message and data rates may apply. Reply STOP to any text message to stop texts, use the unsubscribe link in any email, or tell us by phone. Consent to marketing messages is never a condition of receiving treatment.
Who else sees it
We share information only with the service providers that make the site and our follow-up work — our web host, our form delivery provider, our email and scheduling tools, and our analytics and advertising measurement providers — and only to the extent they need it. We may also disclose information where the law requires it.
Advertising and analytics providers may set cookies and collect technical data through this site under their own privacy policies. We do not send them the content of your form submission, and we do not include health information in any advertising or conversion event.
How long we keep it
We keep enquiry records for as long as needed to respond and to meet our legal and business obligations, and we delete or de-identify them when that period ends.
Your choices
You can ask us what personal information we hold about you, ask us to correct it, or ask us to delete it. Write to info@utahstemcells.com or call (801) 999-4860, and we will respond within the time the law allows. You can also control cookies through your browser settings.
Note for patients
Information you give us through this website is not protected health information. Once you become a patient, the health information we hold about you is handled under our clinical privacy practices and applicable law, which are separate from this policy and which we provide to you at your visit.
Changes
If we change this policy we will update the date at the top of this page.
